INSURANCE INFORMATION
I am on most insurance panels. Prior to your first appointment please contact your insurance carrier to determine your mental health benefits, including your deductible and copay or coinsurance. Also ask if a pre-authorization or a physician referral is required for treatment and if I am an in-network provider.
If I am an out-of network provider, ask the representative about your out-of-network benefit coverage. Such benefits will reimburse for the services you receive from me but likely will require a higher copay or co-insurance payment from you. I do provde sliding scale services for those without insurance.
